\nHumana Healthy Horizons in Ohio and CPESN\u00ae<\/sup> USA are expanding access to care throughout Ohio with the launch of its first Pharmacist Quality Rewards program<\/b>, a value-based care (VBC) initiative designed to improve outcomes for members living with chronic conditions. The program builds on Humana\u2019s existing partnership with CPESN\u00ae<\/sup> pharmacies by financially incentivizing community pharmacists to help close care gaps for members diagnosed with hypertension and diabetes.<\/p>\n

A Value-Based Approach to Chronic Condition Management<\/b><\/p>\n

\nHumana collaborated closely with CPESN\u00ae<\/sup> USA and internal clinical, pharmacy, and population health partners to design a market-specific pay-for-performance model aligned to Humana\u2019s Chronic Conditions Population Health Stream. The program focuses on members managing hypertension or diabetes\u2014conditions often treated with long-term medication therapy and frequent pharmacy visits.<\/p>\n

\nMost members visit their community pharmacy every month, which means they will likely interact with their pharmacists more often than they do with their primary care provider. Through this program, CPESN\u00ae<\/sup> pharmacies receive actionable insights to help identify members with gaps in care and engage them at the point of care.<\/p>\n

\nParticipating CPESN\u00ae<\/sup> pharmacists receive a list of members who fill prescriptions at their pharmacy and may be missing recommended screenings or have out-of-range results. Pharmacists then encourage members to complete appropriate testing\u2014often on-site through point-of-care services\u2014and notify prescribers when results indicate additional follow-up is needed.<\/p>\n
