Springfield Clinic, a community-based primary and specialty care practice with more than 650 physicians and advanced practitioners delivering care across more than 80 specialties in more than 20 counties, entered into a long-term partnership with agilon in 2024 to transition to a full-risk, value-based care model currently supporting approximately 18,000 senior Medicare Advantage patients.<\/p>\n
\nThis expansion builds upon the organizations\u2019 existing partnership and is expected to help reach approximately 21,000 more Traditional Medicare beneficiaries across the communities Springfield Clinic serves. Through agilon\u2019s ACO, which participates in the Medicare Shared Savings Program (MSSP), Springfield Clinic is anticipated to gain additional support to deliver more coordinated, personalized, patient-centered care focused on prevention, earlier intervention and improved outcomes while helping reduce unnecessary healthcare spending.<\/p>\n
\nagilon brings extensive experience supporting physicians through accountable care models. Since 2021, agilon\u2019s ACOs have generated more than $510 million in gross savings, including $125 million in savings to the Medicare Trust Fund, demonstrating the ability to improve outcomes while reducing costs.
