Family Practice Center, central Pennsylvania\u2019s largest independent primary care physician group serving more than 200,000 patients, entered into a long-term agreement with agilon to transition to a full-risk, value-based care model for certain Medicare patients beginning in 2024.<\/p>\n<p>\nThis expansion will help reach more Traditional Medicare beneficiaries across the communities FPC serves.
